Insurance Information

Many health insurance plans cover nutrition visits, often at NO COST to you. Telehealth is also covered by the majority of plans we accept, especially during COVID-19. Telehealth policies may change as soon as May 2023, so please make sure to ask about telehealth coverage when you verify benefits with your insurance. 

Hayat Nutrition Accepts:

Coverage varies greatly between plans. Please scroll down for help with understanding what your plan covers. If insurance declines to cover your visit(s) for any reason, you are responsible for the full amount billed.

If you do not see your insurance plan listed above, you may seek out-of-network reimbursement. Some insurance plans offer out-of-network benefits.

To learn more about your out-of-network benefits, please contact your insurance company directly.

Contact information regarding benefits can be found on your insurance card, usually listed as a phone number for “member services” or “customer support.”

Once you have verified out-of-network benefits, we can provide you with a super bill that you will submit directly to your insurance provider for possible reimbursement.  Please note: payment is due at the time of service for patients who are not members of insurances we are in-network with. 

Payment

We accept  Zelle, credit/debit cards, HSA & FSA cards. An active credit/debit card on file is required to schedule appointments. If insurance declines to cover your visit(s) for any reason, you are responsible for the full amount billed.

Before your visit, please call your insurance company to inquire about your nutrition counseling benefits.

We have listed some information below regarding insurance coverage for nutrition therapy based upon our knowledge and/or experience to date with the companies.

Please understand that we absolutely cannot know the details of each plan and that the information below is not a guarantee of coverage. If choosing to use insurance benefits, it is your responsibility to understand your plan and its deductible.

While we ALWAYS recommend calling your insurance to check benefits for nutrition counseling, most plans DO cover visits with us.

A doctor’s referral is typically not required, unless you have an HMO plan. 

Fill out THIS form with your insurance information and we’ll let you know if we identify any issues with coverage. Doing this is NOT a replacement for calling your insurance and asking the questions listed on the script provided above. 

We are in-network with this insurance.

While we ALWAYS recommend calling your insurance to check benefits for nutrition counseling, most plans DO cover visits with us.
